Participant Waiver and Release of Liability

Introduction

The Mammoth Slip and Slide is an activity where participants slide down an
inflatable water slide. This waiver must be accepted before participating. If you are
signing on behalf of a minor, it is your responsibility as a parent/guardian to explain
this document to your child. A safety briefing will also be provided on the day as a
refresher.

Acknowledgement of Risks

I understand that there are risks associated with participation in The Mammoth Slip
and Slide and activities within the event site. Multiple users sliding at the same time,
with or without equipment, increases the risk of injury. I agree to follow all posted
signs and verbal instructions from staff at all times. I understand the nature of the
activity and accept that not all risks can be eliminated.

Potential Injuries & Medical Conditions

All participants must be confident in water. Risks include but are not limited to: head
injuries, sprains, burns, cuts, bruises, drowning or near-drowning, and other
accidents from slipping, sliding, or falling. Serious injuries could include permanent
disabilities, spinal injuries, heart attacks, strokes, or death.

Participants may come into contact with chlorinated water, mud, and other substances which may not be
tested for chemicals or illnesses.
The Mammoth Slip and Slide and its staff cannot be held responsible for illness or
disease transmission. I confirm I am fit and healthy to take part and have consulted a
doctor if necessary. Any medical treatment required due to participation is at my own
expense.

Assumption of Risks & Indemnity Agreement

I acknowledge that participation in The Mammoth Slip and Slide involves inherent
risks, including the risk of injury. I accept full responsibility for any risks, damages,
liabilities, or losses that may arise from my participation in this activity.
By taking part, I voluntarily and irrevocably waive, release, and discharge The
Mammoth Slip and Slide, its directors, staff, volunteers, and representatives from
any and all claims or causes of action arising out of my participation, except in cases
of personal injury or death caused by their negligence.
I further agree to indemnify and hold harmless The Mammoth Slip and Slide and its
representatives against any claims brought by myself or others in relation to my
participation in the activity, except where such claims arise from their negligence.

Severability

If any part of this agreement is found to be invalid, the remaining sections shall
remain in effect.

Event Cancellation

No refunds will be issued if the event is cancelled, postponed, or modified due to
weather, acts of God, or other uncontrollable factors.

Participant Dismissal

I understand that I may be removed from the event, without refund, if I endanger
safety or negatively impact the event, others, or the premises.

Supervision of Minors

Parents, guardians, teachers, or group leaders must supervise children under 18 at
all times and are fully responsible for their safety.

Minor Participation

If signing on behalf of a minor, I confirm that I have explained this waiver to them and
they understand the terms and their responsibilities.

Acknowledgement of Understanding

I confirm I have the authority to agree to these terms for all participants I am booking
for. I understand that by accepting the waiver during the booking process, I am
giving up certain legal rights voluntarily.
